**Sports Clearance Forms for ALL Sports are due EVERY season.
Students wishing to participate in a Spring SPORT: MUST have a current physical on file dated on or after 03/01/11; MUST: have a completed current Sports Clearance Form. Must have: Self Carry medication orders from their Pediatrician for all medication needed.
Example: Inhalers, EpiPens, Motrin, Tylenol.

**Sports Clearance Forms (available from this page under Related Files) for the All Sports Seasons (2010-11) must be signed and dated( according to the date indicated for each season) and return to the Health Office.

NYS Student Health Requirments
1. Physicals / Immunizations:
NYS MANDATES all 7th graders to have on file a current physical dated on or after 9/01/10 For the 11-12 school year.
Please ask your Doctors Office for a copy of any new Physical / Immunization forms as the student recieves them for all grade levels. Turn into the Health Office.
September - All 6th grade students born on or after 1/1/94 must provide evidence of receiving the Tdap Immunization for school attendance. If a student does not turn 11 years old prior to September a MD note indicating the student will receive the TDAP Vaccine within 30 days of there 11th birthday is required.
NYS Required immunizations are:
Tdap for incoming 6th graders
Diphtheria
Oral Polio
Measles, Mumps and Rubella (MMR)
Tetanus.
Hepatitis B vaccine is mandated for children born on or after January 1, 1993, and all students entering 7th – 12th grade.
2. Medications:
According to NYS law, ANY medication either administered and/or self carried at school, including over -the -counter medications (Cough Drops, Motrin, Tylenol ) require written permission from the physician and a parent or guardian.
Medications must be in original contanier.
Medication is not provided by the Health Office. Must be supplied to be administered.
3. Allergies:
Students with food / Insect / latex / or any other allergy requiring the use of an Epi pen - must have an emergency care plan filed with the school nurse.
