Students wishing to participate in a Spring Sport MUST have a current physical dated on or after 03/01/09 and MUST have a Sports Questionaire ( which is completed for EACH season) on file at the BRMS Health Office. A self-carry medication order signed by the doctor for all medication(s) needed.

Sports Clearance Questionnaire for the Spring Season must be signed and dated no earlier than 02/08/10 and returned no later than 02/24/10 Spring Sports Start Dates: All Varsity/JV Sports 03/08/10 Modified B Sports 03/20/10 *Any Student trying out for Varsity and/or JV must go throught THE SELECTION CLASSIFICATION process. Contact the Athletics Office 267-1062 ASAP!!!!!
NYS STUDENT HEALTH REQUIREMENTS
1. Physicals/Immunizations:
- NYS MANDATES that all 7th graders have on file a current physical dated on or after 9/01/08 For the 09-10 school year. Please provide the Health Office with a copy of any new Physical forms as the student recieves them for all grade levels.
- Please ask you Doctors Office to record all Immunization given and turn into the Health Office.
- September - All 6th grade students born on or after 1/1/94 must provide evidence of receiving the Tdap Immunization for school attendance. If a student does not turn 11 years old prior to September a MD note indicateing the student will recieve the TDAP Vaccine within 30 days of there 11th birthday is reguired.
- The required immunizations are Diphtheria, Oral Polio, Measles, Mumps and Rubella (MMR) and Tetanus. The Hepatitis B vaccine is mandated for children born on or after January 1, 1993, and all students entering 7th – 12th grade.
They must have completed one of the two recommended schedules for immunization or provide serologic evidence of immunity against Hepatitis B infection. The Hepatitis B Series consists of a 2 or 3 shot schedule and can take over 6 months to complete.Varicella (chicken pox) is required for children born on or after January 1,1998. 2. Medications:
According to NYS law, ANY medication either administered and/or self carried at school, including over -the -counter medications (Cough Drops, Motrin, Tylenol ) require written permission from the physician and a parent or guardian. Medications must be in original contanier. 3. Allergies: Students with food / Insect / latex / or any other allergy requiring the use of an Epi pin - must have an emergency care plan filed with the school nurse.
