Sports Clearance Forms for Spring Sports may be found in several places--on the District Athletic site, School Nurse site(Click on News and Info to the left) or outside the Nurses's Office in the black bin,,beginning January 20,2012. Sports Clearance Forms should be signed and dated no earlier than February 1,2012 and returned to SHS Health Office by February 10,2012. You must have a current physical dated on or after 3/1/2011 on file in this Nurse Office. Please drop off  all completed forms to Irene Steele, School Nurse in the Health Office.Please do not give your forms to anyone else to avoid being lost or displaced!If you are turning in forms after February 10,2012, I cannot ensure your student-athlete will be cleared in time for first practice!!!  Thank you, Irene Steele,RN,BSN,SN
