Sports Clearance forms for Fall sports may be found in several places-on the District Athletic web page link below,this School Nurse site(see link below)can be picked up at the Sutherland High School Main Office, or outside the Health Office in the black middle bin,beginning June 1,2010. Sports Candidate Questionnaire forms should be signed and dated no earlier than 7/16/10 and returned to SHS by 7/30/10. You must have a current physical dated on or after 8/1/09 on file in this Health Office.Please drop off completed forms to the School Nurse in the Health Office or the Main Office at Sutherland High School(there is a sports box).
