Sports Clearance forms for Spring sports may be found on the district athletic web page link below or at the Sutherland High School Main office beginning Feb.1,2010. Sports Candidate Questionnaire forms should be signed and dated no earlier than 2/8/10 and returned to SHS by 2/24/10. You must have a current physical dated on or after 3/1/09 on file in your school's health office.Please drop off completed forms to the School Nurse in the Health Office.Thank you, Irene Steele,RN,BSN,SN
