Sports Clearance: Go to Athletics and follow directions for Family ID. Process is completely online. All Physical/Medication Order and CarePlans must be upload. No hard copies are accepted. Family ID may be reached at 1-888-800-5583.
Grade level mandatory Physicals: 9th /11th and 12th Grade Meningitis Vaccine documents may be faxed 218-1181,emailed or dropped off in the main Office at SHS. Medications order are needed every year and may be delivered as above.
NYS Student Health Requirements:
1: Physicals / Immunizations**** NEW NYS REQUIREMENTS 2019-2020 SCHOOL YEAR*****
NYS Mandates all 9TH & 11TH graders MUST have a current physical dated on or after 09/01/18 for the school year 2019-2020.
Please ask your Doctors Office for a copy of any new PHYSICAL / IMMUNIZATION forms as the student receives them for all grade levels. Please turn into the Health Office.They maybe faxed 585-218-1181,Emailed or dropped off at the Nurses Office. This process keeps student physicals up to date and allows for complete health care, as well as easier sports clearances & working papers. New Students to the District must have a NYS physical on file within 30 days of enrollment.
For the 2019-20 school year:
- One dose of meningococcal vaccine is required for students who will be in 7th, 8th or 9th grade. If your child had the first dose before 7th grade, then another dose is not required until 12th grade.
- Two doses will be required before 12th grade. Most students entering 12th grade got their first dose when they were younger and are now due for their second dose, or booster. This booster is needed because protection from the vaccine decreases over time.
- A small number of teens who received two doses before their 16th birthday may need a third dose on or after their 16th birthday in order to enter 12th grade.
- The only teens who will not need a second dose before 12th grade are those who got their first dose on or after their 16th birthday.
It's best to check with your doctor to see whether or not your child needs the vaccine. Students who are not up-to-date will not be allowed to attend school until they are vaccinated.
According to NYS law, ANY medication either administered and/or self carried at school or sports including over -the -counter medications (Cough Drops, Ibuprofen, Acetaminophen) require a written permission from the Physician and Parent or guardian. Please be sure they are in the appropriate bottles
Students with food/insect/latex or any other allergy requiring the use of an Epi-Pen must have an emergency care plan on file.
Reasons not to send your student to school:
Vomiting and /or diarrhea in the last 24 hrs. (Students must be symptom free for 24 hours to return to school)
Fever 100.4 or above without medication in the last 24 hrs
Dx Strep throat. Must be on medication 24/48 hrs min to return to school.
