Please open and save to your computer the Recommendation Information Sheet..  Return it to me ASAP. I write the letters in the order in which I receive these completed forms so please be attentive to your due dates. You can submit it by email or drop it off to me at school.

You do NOT need to have your colleges selected, or pink sheet and applications completed to begin the process with me, just leave those portions blank for now. Your submitted Recommendation Information Sheet will initiate the process and you can bring me the other related documentation and/or let me know when your Naviance Account is established as you progress.

Please include the following information to assist in the processing of your College Recommendations.  You may submit components on a rolling basis.

It is wise to verify with each college's admissions office just prior to deadlines to confirm that they have received all of your required application paperwork including my recommendation.
