Please see below for information regarding the 2021-2022 school year.

    Physicals and Immunizations
Physicals completed by a doctor are required for new students and all students entering 9th and 11th grades. New York State requires they be on file in the Health Office within 30 days of the new school year for admittance to school for new students. Physicals dated after 9/1/2020 may be accepted for the 2021-2022 school year. 12th graders are required have a minimum of 1 Meningitis vaccine on or after the age of 16 years. All immunizations must be up to date within 14 days of starting school.

**Special Note for 10th and 12th graders**
Due to COVID-19, the physical requirement deadline has been pushed out to the Fall of 2021. If your student is in 10th or 12th grade and you did not turn in an updated physical dated on or after 9/1/2019, you also have until September 2021 to turn these in. An email was sent to all parents/guardians of students in June 2021 to those with outstanding physicals.

    Medications
According to NYS law, any medications administered at school, even over-the-counter medications (cough drops, Tylenol, Advil), require written permission from the physician and the parent or guardian. They must be unexpired, in their original container, and must be brought to school by an adult to ensure safety. Inhalers need a pharmacy label on the inhaler itself, not just the box. Medication orders including epi-pens, inhalers, other self-carry orders, and daily medications must be renewed each fall. Self-carry forms can be found here.

**Over the counter medications such as cough drops, Tylenol, or Advil are not stocked in the health office. We do not accept forms with standing orders**

Injuries, PE Excuses, and Elevator Passes
If your student needs to be excused from Physical Education, a note from their physician describing the injury and the activities allowed is required. If an elevator pass is needed, the physician must also note that on the excuse. Our fax number is 218-1181 if needed.

Documentation can be emailed, faxed, or dropped off at the main office.

    Sports Clearances:
For the most updated information on sports registration, please visit the Athletics webpage. All registrations are completed online through FamilyID. If you need assistance uploading your physical, please reach out to FamilyID at 1-781-205-2800

    Working Papers
Working paper forms can be picked up at the counseling office or found on the counseling webpage. In order for our staff to sign off on the Certification of Physical Fitness, your student must have a physical on file with us that has been completed within 12 months prior to signing. During the school year, the form can be signed by the school nurse. During the summer, the form will be signed by counseling staff and they will have our most up to date list of physicals completed before our nursing staff leaves for the summer.

Please do not send your child to school if they are experiencing any of the following:
-Vomiting or diarrhea in the last 24 hours. Students must be symptom free for 24 hours before returning to school
-Fever of 100.4 or above. Student must be fever free for 24 hours without medication before returning to school
-Diagnosis of strep throat. Student must be on medication 24-48 hours before returning to school.

    Latex Allergy Alert
There are students with severe latex allergies. Please DO NOT bring any latex products into this school. Some items that can contain latex are: rubber gloves, medical products, latex balloons. Thank you.
