Updated NYS School Vaccination Legislation
New York State legislation (Public Health Law 2164) has eliminated religious, non-medical exemptions from school vaccination requirements, effective June 13, 2019. The revised law specifies that children in a public, private or parochial school, without a medical exemption, must receive the required immunizations to attend or remain in school. The District will be required to exclude a child from school if these provisions are not in place. If you have questions about immunizations, please call your school nurse.
Immunization Requirements
Please ask your doctor’s office to record all immunizations when they are given. The required immunizations are diphtheria, oral polio, measles, mumps and rubella (MMR) and tetanus. The Hepatitis B vaccine is mandated for students born on or after January 1, 1993, and all students entering 7th -11th grade. Varicella (chicken pox) is required for students born on or after January 1, 1998; all students born on or after January 1, 1992, who transfer to a NYS school from another state or country; and all students born on or after January 1, 1994, who enroll in 6th grade.

Health Appraisals - Physicals
New York State Education Law requires each student entering Kindergarten, and in grades two, four, seven and ten to provide proof of a physical examination with updated immunizations. You may download and print a copy of the health appraisal form by clicking the link below. This form must be completed by the physician. Acceptable physicals are those dated from January of the current calendar year. Completed forms should be returned to the School Health Office following your child’s physical examination.
