Students wishing to participate in a SPORT 2019-2020 school year.
1. MUST GO ONLINE TO ATHLETIC PAGE TO FAMILYID TO REGISTER FOR ALL SPORTS. Please see link below on Athletics home page.
2. Must have a current physical on file at BRMS on or after 8/1/2018. Please be advised, the physical date will change for every new sport season.
3.Must have Self Carry medication orders from their Pediatrician FOR ANY MEDICATIONS NEEDED.
- Example: Inhalers, EpiPens, Motrin, Tylenol. ( If your child does not require medication disregard step 3)

New York State School Mandates as follows
1. Physicals / Immunizations:
NYS MANDATES all 7th graders to have on file a CURRENT PHYSICAL EXAM dated on or after 9/01/2018 For the 2019-2020 school year.
Please ask your Doctors Office for a copy of any new Physical / Immunization forms as the student receives them for all grade levels. Turn into the Health Office.
2. IMMUNIZATION REQUIREMENTS FOR STUDENTS ENTERING 6TH GRADE FOR 2019-2020 SCHOOL YEAR
6th graders that have not submitted proof of immunization to the Health Office according to the guidelines below will be excluded from school in September.
TDaP Students who are entering 6th grade and who are 11 years of age or older must receive TDaP vaccine before entering 6th grade. (This vaccine is not the same as the DTaP administered before age 7 years old).
Students who are entering 6th grade and are 10 years old will be permitted to start school without Tdap vaccination, but must be immunized within 2 weeks of their 11th birthday.
VARICELLA Students entering 6th grade must receive 2 doses of Varicella (chicken pox) vaccine before entering 6th grade, regardless of age. The 1st dose must be given on or after their 1st birthday in order to be valid.
A child does not need to receive these immunizations if the parent submits a statement signed by the child's Health Care Provider which states that the child has had chicken pox disease.
POLIO Students entering 6th grade must follow the new interval spacing recommendations according to the current ACIP guidelines for Polio vaccination (IPV)
Student are required to receive 3-5 doses, with a minimal age for the final dose of 4 years old
Students who require another dose must receive this immunization before entering 6th grade, regardless of age.
MIDDLE SCHOOL IMMUNIZATION REQUIREMENTS
*The following immunizations must now follow the ACIP recommendation for interval spacing between doses. Some students may need additional vaccinations to satisfy this new NYS mandate. Note that only 6th graders are mandated to follow these guidelines for Polio and Varicella for the 2019-2020 School Year as stated above.
Diphtheria* 3 doses
Polio 3 dose
Measles, Mumps & Rubella* (MMR) 2 doses (minimal age for 1st dose is 1year old))
Hepatitis B* 3 doses
Varicella (Chickenpox) - 2 dose (minimal age for 1st dose is 1year old)
Tdap 1 dose (11 year old students before entering 6th grade)
Meningococcal required for school entry as of September 1, 2016 for 7th graders.
If your child's physician is unable to vaccinate your child before school starts, you may bring your child to the:

Shots are free for all children through age 18 years. However, if your child is covered by any health insurance, please bring the benefit card with you. A parent / guardian must be with children at all times.
Please bring your child’s immunization record and any requests for vaccination that you have received from your child’s school.
According to NYS law, ANY medication either administered and/or self carried at school, including over-the-counter medications (Cough Drops, Motrin, Tylenol ) require written permission from the physician and a parent or guardian.
Medications must be in original container.
Medication is not provided by the Health Office. Must be supplied by the parent to be administered.
Students with food / Insect / latex / or any other allergy requiring the use of an Epi pen - must have an emergency care plan filed with the school nurse. This needs to be done for every new school year.
Students must be symptom free from diarrhea and/or vomiting and/or fever without the use of fever reducing medications for 24 hour before returning to school.
